I'd like to try a weekly online questionnaire/survey with students where I could ask them about the class each week, including content, relevance, how they feel about their progress, thoughts on the teaching etc. I would want it to be anonymous. I could make a paper questionnaire but have heard of people doing this through online tools/software. Does anyone know of any online sites that I could use for this?

Hi,
You could google "needs analysis surveys and ESL students" for examples but naturally will be tailored to your students and course. You could also occasionally give students a "reflection" visual at the end of class (e.g., a large light bulb): What questions do you have about today's lesson? You can also monitor understanding by using this visual for a writing prompt such as: "Take 5-7 minutes to write about 2-3 important things you learned in today's [grammar/writing/reading/] lesson. These are beneficial to reticent students who hesitate to ask questions in class.

Dragonpiwo,
I did not intend to post that 2nd message with reference to learning styles. In any case, I am aware of all the empirical research regarding learning styles. However, based on 35 years of teaching experience and having pursued graduate degrees/research in Psychology, Applied Linguistics and TESOL, there is some merit to this claim. When I ask students how they best learn, they do express preferences. I know myself that if a professor presented material auditorily only, I would be lost. Many of my Asian students are more visual than Arabic learners IN GENERAL. However, as you know as a seasoned TESL/TEFL teacher, we present material multi-modally to accommodate all learners.
