Just wondering what are the options as most are on tourist visas.

This is a good question. I bet that if you went to the Caja (health system) and said you wanted to sign up and pay them something each month, they would not turn you down. I might be wrong. You can also go to a private doctor and get pretty good service for relatively cheap.
