Anybody know the name of this form in Korean? Is there a universal standard, or do I just need something printed up on my school's offical stationery?

Korean translation of Certificate of Employment is 재직 증명서. Ask for that and your employer (past or present) is to immediately produce one and supply it to you. Make sure it has the red stamp on it.

You can produce one and give it to your employer to fill in and stamp or they can produce it for you. I would recommend that it be in both korean and english.

Do either of you know if you should have to pay for the certificate? I've just requested one from my school, and they've agreed to it (as they should,) but mentioned it will cost me ... 400 won. It's not the money obviously, just the principle. Anybody know if this is legal?

Yes, they can ask for it, my old school did also...

They have to ask for it - it is a legal thing. Plus, why are you even questionning 400won?
