Portfolios for Academic English Prep

I am working in the English preparation dept for a university. All of the students are taking intensive classes to prepare for entrance into their departments. They take classes in each of the four-skill areas and grammar, and the 4-skill classes are content-based. We are currently working to instate a program porfolio as a part of their assessment. The students will have the option to do either web- or paper-based Portfolios. I'm familiar with portfolios as an assessment tool, but rather than re-inventing the wheel, I'm wondering if anyone has any information on the following:

1) Sections/General content for university-level EFL portfolios
2) Portfolio scoring rubrics for each of the 4-skill areas

Any ideas would be greatly appreciated!
