"The heavyweight champion is set to make his comeback."
1. Have all preparations been made for his comeback?
2. Has he decided to make his comeback?
3. Any other meaning?
Thank you.

"The heavyweight champion is set to make his comeback."
1. Have all preparations been made for his comeback? Yes
2. Has he decided to make his comeback? Yes

You've pretty much got it, another way to phrase this might be:
The heavyweight champion is ready to make his comeback

I can't think of any other meanings.

It could also mean he is determined or resolved to fight again.
