I just need a good level test for my adult students it should include the following. Thanks.
1. speaking skill questions
2. comprehension skill questions
3. grammar skill questions |

Depending on what level you think they should be at...
find (google is your friend) a copy of the Cambridge FCE test or a buy a copy of an IELTS test prep book.
They will have tests that you can use for general English proficiency testing (4-skills) and you can google for a scoring rubric.

And that is different from other "English proficiency" how exactly?
Both sets of tests are easy to find, easy to administer and give a level from zero to advanced.
The FCE or an IELTS prep test from a book/CD are pre-canned so they are pretty much idiot proof in terms of a teacher who has no real clue about assessment and evaluation* administering them and getting a reasonably accurate level.
* do the terms consistency, validity and reliability have any meaning in your lexicon? They are all met by using a standardized test that is easy to find and easy to administer (they take about an hour-90 minute for accurate 4-skill testing)
