anybody have the RPR instead of the VDLR done ?

my doctor says the VDLR is outdated and less effective.

everyone knows the sacm hasn't updated the form in decades

what to do?

That's acceptable. On my form, I indicated both TPPA and RPR diagnostic test results for syphilis under "Venereal diseases." Keep in mind you'll be sending in your test results anyway as part of your employment visa application packet.

By the way, the Saudi government and not the SACM is responsible for determining the requisite lab tests.

Your doctor should essentially check both tests negative. I had only one test done, the one that they would perform normally. It was enough to pass through immigration.

When I first took the form to my doctor, he was shocked to see such intrusive tests just to obtain employment visas. I am quite certain that most of the diseases and viruses mentioned in the form are found in KSA and other third world countries.

Sure, but keep in mind the world is global. Ironically, considering the profession we're in, some of those lab tests make real sense if the job applicant recently returned to the US after a teaching stint in countries where those diseases are prevalent.
