I have been here longer than you and also don't know where to find the forms. I have never needed them.

Yeah I've only ever had them sent by the school.
But just to clarify, I'm giving a talk to a group of people considering looking for work in China. The forms will make nice handout and expose them to some Chinese doublespeak.
That said anyone got the contract blank?
