I don't know where to get the form and the sticky doesn't work and no one seems to know what it is at the doctor's office.
The Chinese Embassy site didn't even say that I need a certificate to apply for the work visa....
Ideas? |

Do a Google search for 'physical examination record for foreigner' and you'll get links to a load of Chinese embassy sites that have it. Then print it out, take it down to the doctor's and watch them chuckle at the odd disease names. Easy. But do get a nice big official stamp on it with the signatures, that's the main thing. |

I just went through the same thing. I am in Canada and the Chinese Embassy web site was useless. I phoned my travel agent ans she said that a physical was no longer necessary to get the "Z" visa.
So I filled out the basic form for the visa and sent it in with my passport and work papers... and a week later I have my visa.
Not sure if you are in Canada or not, but it isn't needed here. |
