Hi Qiang,
I haven't done that, but I've recently learned that whether you need the physical depends upon the type of visa you get, and that'll depend upon where you work.
The Z visa requires a medical exam, and even if you get one in China, it's virtually a given that they will insist that you get another one done here. If you're concerned about, say, blood work being done with bad needles, have a doctor at home give you a prescription for what are known as (I believe!) 'butterfly needles'. They're small, and with the prescription nobody will give you a hard time about it even if they notice them.
The F visa is more of a 'consultant' visa, and can be obtained in 3,6,9 and 12-month varieties, and will let you work just fine, get a residence, etc. This type is a lot easier for many schools to obtain, and is becoming fairly popular -- and it does not require a medical exam. Be aware, however, that the F does not allow you to bring family over (which the Z does), and I don't know if you're legally allowed to exchange money for foreign currency on the F. It's obviously do-able, but I'm just unfamiliar with that aspect of it.
So, that doesn't directly answer your question, but I hope it helps! Good luck!
Cheers,

(EDIT: A few other differences exist that are important to note, I think. The Z visa indicates a contract between you and your school, whereas the F is indicative that you're consulting for whoever you happen to be working for. Because of this, you can leave your school at any time on an F visa without a letter of release.... but the school, likewise, can end your services at any time. Be aware!) |
