I am not sure if you can find a test with English instructions, though some do have them where I am in Tokyo. But there are test kits in almost every drug store. Just look in the section where women's health products are sold and you will see them. Don't mix them up with an ovulation test kit which is usually sold in the same place and looks almost the same--they usually come with several tests--like 8 or 10. A pregnacy test kit will only have one or two in a packet. I would say, go for 2 tests just to be sure. They are very good tests here, very sensitive and almost 100% accurate. You can see the telltale line or dot come up almost immediately.
Being the mother of two children both born in Japan I have used them many times. My Japanese is terrible but they are so easy to figure out. There are very clear pictures. If you want really detailed instructions you can PM me. I hope you get the result you are hoping for,

a friend of mine bought one at random in an Osaka Chemist and she found English instructions inside it...after she panicked and called a Japanese speaking friend over...

I bought one in a normal pharmacy and it had English both on the outside of the package and the instructions on the inside. This was in Saitama, but I saw them everywhere all over Tokyo. |

OP,
it depends on where you are i guess...if you're in a big city, you might be ok with finding a preg. test in english. I'm in a really small city which only had 2 types of preg. tests, both of which were in japanese. i had no problems with this at all, and my japanese is non-existent.
Look for the one that has pictures, you should be able to follow them to find the answer.
