Greetings fellow teachers,
I will be travelling overseas soon for a vacation. I've been doing some research on the National Health Insurance here in Japan which we are all obligated to pay for on a monthly basis. It turns out that we could get reimbursed if we get ill overseas:
"If you pay for medical treatment for illnesses or injuries incurred while traveling overseas. (You must take a stipulated form to be completed by the doctor overseas.)"

Does anyone know a link online to download those 'stipulated forms'? It think it would be wise to take some with me to be safe. |

Looks like they're all here. If you can't read it, there's a bunch of PDF files linked. You need the first and the second for a normal doctor visit, the second and the third for a dental visit, and the fourth one is some kind of key that I guess you need to find the code for your problem on.
The second bank of files are for official translations of the other documents. They are for if you choose to get it translated personally. It says you can also submit a written request for translation at the time of submission.

edit: Maybe you need all three for the dentist. It's not really clear on that. |
