This is a common challenge for many Japanese students. Have you tried explicitly teaching conversation strategies (follow-up questions, circumlocution, word coinage, re-phrasing, non-verbal cues, etc.)?

Check out the link below to a textbook covers them and gives you lots of ideas for activities and games to re-enforce and practice. You should be able to order a free inspection copy from MacMillan Language House.

what about worksheets?

the mother said no textbooks, OK. But as you say its hard to have a normal conv for 40 mins all the time.
A worksheet (of some kind) would be a good balance, it provides some framework for a conv / discission but not like a hard and fixed textbook.

I can't say for sure this would work, but I find students open up when they learn a new skill; something where using English isn't the focus of a "lesson" but simply a by-product.

A new sport for example, although that might not work in your case. Maybe she would respond to discussing a book or movie you could give her?

Thanks for all the replies!!! I talked to my boss, and we're gonna try some different things... including asking the mom to let us use a book... anyway, I'm going to make a new post about this... thanks for all the help!!!
