I'm going through the same thing here. My school was confused about where and what kind of medical exam I should get. They asked me to call the consulate, who didn't know either, and told me to ask my school. I think I found the right form, but even though I got a physical like two months ago, my university clinic won't let anybody just transfer the info over onto the form -- I need to go through another exam. Luckily I can get an AIDS test free through the clinic, but all of this is a huge pain in the butt.
This was all more simple before ... |

That's the old form. The new form has specific blood profile requirements:
(Serodiagnosis)
HIV-Ab(I+II)
RPR/TPHA Syphilis
HAV-IgM Hepatitis A
HbsAg Hepatitis B
HcV-Ab Hepatitis C
SGPT Serum glutamic pyruvic transaminase, an enzyme that is normally present in liver and heart cells. SGPT is released into blood when the liver or heart are damaged. The blood SGPT levels are thus elevated with liver damage (for example, from viral hepatitis) or with an insult to the heart (for example, from a heart attack)
I was sent a copy by my recruiter, but it took me a long time to find it online. Then it disappeared from the Washington, DC Chinese consulate website. All that I have found is the old form, so I suppose that's still the needed form. The main difference is that the serodiagnosis tests are spelled out. If your doctor reads the old form carefully, he'll perform the needed blood tests.
Anyone needing or wanting this form can PM me with their email address and I'll send it along. As I said before, this was sent to me by my recruiter. Then I did some double checking, found a PDF with the exact same form, then the form disappeared. |

the yunnan university emailed me a pdf file, which was identical to the
link posted.
the consulate gave me a poorly-photocopied copy of what appears to be
an even older version. it requires the same tests, but the formatting is
less professional. |
