Laughing Shijiazhuang is an absolute dive but at least it's easy to get into Beijing for a weekend

Lanzhou - that's somewhere even worse.

This! But this is kind of hard to determine until you arrive. The number of subway lines, or the lack of a subway, is also usually a pretty good indicator. Lots of lines usually means first tier. Over the past few years, China has been making big pushes for subways in its second-tier cities, so if it has one or two lines open or about to open, it's probably second-tier (Dalian, Zhengzhou, e.g.). Third tier won't have a subway at all, but will likely be within an hour or two bus ride to a third-tier city. Fourth tier won't even be on the map. I've lived in second and third tier...I really wouldn't recommend third tier or below unless you're really adventurous. Third tier means difficultly doing anything without speaking Chinese, including going to the doctor, and medical care will largely be traditional Chinese medicine. Even a few Western food options likely won't be available at grocery store, and there might not even be a movie theater or arcade, even if the population is approaching 1 million.
