I got a job offer from a Uni, signed the contract, and got an email from the FAO office telling me what they needed from me. I was told a letter of recommendation from former boss, my health check, a non criminal background check, and that I needed to send original documents to them in the mail. Then they will send me the form I need to get my visa.
Time is of the essence here, and I need more specifics on how to do these things. I have sent emails back for 5 days with no response. I only sent 2 emails bc I don't want to push it and figure they're not on complete regular schedule yet, and not checking email.
Here's the issue. I have no forms or no clue how to get a health check or a non criminal check. I also don't know exactly what the letter of rec needs to say or if that needs to be original. My last boss is really flaky and to get him to send something in the mail could be tough. Can I use a former boss? Can the letter if rec just be emailed and printed out? I am losing time and was hoping someone here could help me get the ball rolling til the school gets back to me.

Your school's admin staff is probably on regular schedule now. Classes start next week and they usually return early. If your boss or former boss is too flaky to write a recommendation letter just write something up yourself, stating your duties and how you carried them out, one or two paragraphs, and ask him to sign it. If he won't sign it then do it yourself. I guess you can either email the letter or fax it if they want a signature. Email is probably fine. Ask the school, they're the ones who want it.
If the school's not getting back to you, I remember a thread last week where you wrote that you were thinking of continuing your search because you weren't sure you wanted to go to that small city. Sometimes if you don't decide or get things done fast enough they move on without you. Email them again, you've got nothing to lose. |

So I don't need blood tests or anything? I had a physical exam by a doctor in May, for my summer job, is it possible that I could get that Dr to fill this form out from him seeing me in May? I'm travelling now, and have moved around a lot. I don't have a regular physician. |

I remember you specifically mentioned you wanted to keep looking, sorry for being pedantic but you did write this last week:

That was way before I signed a contract. That was the first offer I had.

That was way before I accepted the offer. This was the first offer I got. |

first paragraph in that link sez:
"...The original form of Foreigner Physical Examination Form (Including blood test reports for ALT,AST,TRIG,CHOL,BUN,CREA,HIV,Syphilis and Hbsag) is required for...."
blood test(s) is the most important part. read the sticky:

sure, why not? you don't gotta turn in the lab results, or the xrays, or the
ekg.....just the form. if your doctor will sign and date the form you're
good. no xrays? no ekg? don't matter. doc can write 'normal' or
'not indicated' in the blank. see, they really just wanna know you don't have
no icky std's. the rest is filler. you're gonna redo the whole thing once
you arrive anyway. now this is important.....the doctor has to use an
official looking stamp on the form. red ink is nice. they've also added a
requirement that the photo has to be 'sealed.' some hospitals have an
embossing seal that would look lovely.
another new requirement....the non-criminal check. try the local cop-shop
wherever you are, see if they can give you a report of no offenses within
their jurisdiction. |

I may have to just get a new med check where I am. It was just a small Dr's office where I got the med check, and I don't think he's hoing to have an official stamp. The criminal check would be done in a town I just got to. Can they do a national check? I have never been arrested or anything anywhere.
Also, is there a form for the criminal check? Or can I just go to the local station? |

well, bro, to be honest, i think you're screwed. you've spent the last
couple months procrastinating, now you're in a rush, and you just don't
have time to get the documents you need. shoulda taken one of those
offers sooner.....or at least started preparing. now you've finally signed
and contract, and you've got nothing ready.
end of august now.........let's say it takes a week to ten days to get all
your documents together and sent off. another 3 days express mail to
china. fao will need to get various signatures at the school, couple days
at least, then apply for work permit and invitation letter, another couple
weeks, then another three days express mail back to you. i assume
you're nowhere near a consulate........so express mail to a visa agent,
couple days visa processing...express mail back to you.

I know my classes don't start til Oct 9. I'm going to do the best I can. Not going to give excuses why I am where I am at this time, but I do have good reasons, and have done the best I can. |

I can share my recent experience with the health exam for Sichuan province. I sent a form that looked almost identical to the one contained in davelister's earlier link. I had the clinic use their own stamp on every single page (I included my blood work and EKG to be safe), and then I also had to get the thing notarized. Fortunately the clinic had their own notary, and again I asked EVERY page be notarized. My school was also very specific about having the photo stamped (I can understand fraud risks, but good grief), so I made sure they stamped/notarized the edge of the photo on the front page. From what I've been told, this was enough to get the authorities to approve my documents and my work permit is currently in progress.
From what I've read in other threads about background checks, a letter from your local authories with some sort of stamp/notariziation should be sufficient. I imagine they're familiar with these requests and already have a standard format.
Speaking from experience, I would suggest requesting two copies of each original doc you plan to send just to be on the safe side. I had to send my health exam off twice before it was accepted. Hope that helps. |

Bear in mind that most of the Chinese schools, government offices, consulates etc. will be closed for more or less the entire first week of October due to a couple of big assed public holidays and festivals. This means you'll have to have all your documentation and visas arranged by the end of September if you wish to make it by Oct 9th.
Just something to be aware of if you don't already know. |
