Hi I posted this in the medical thread but really need an urgent answer. I've got a question about the version of the form I have been sent. The school who offered me the job sent me a copy of the form which is different to any other version of the form I've seen. I turned up at a private medical clinic in the UK today to get it filled out and the doctor had a different form. The issue is in the blood test section. Mine says as well as HIV and Syphilis that I need RPR, PTTA, HbsAG, Anti HPC and ALT GPT. The form the doctor had costed it on only had HIV and Syphilis and I also found a form asking for some of the other things mentioned but not all of them. My question is what form should I do as the one I've been sent with the extra blood results requested adds on another £70 for those tests? |

I just got my GP to fill out the basics on the form to pass me fit to work in China. This sufficed but every case is probably different.
You will need to do another medical once you arrive in China anyway, chest xray, blood tests, etc, which your school should pay for. |

I know the form you mean, it's the standard one that you need to fill out for the Z visa I believe. On the one I currently have they ask you to attach the results of the blood tests to the form, so I'd be pretty careful about missing out any of the required blood tests, it may come back to bite you.
In saying that, there doesn't seem to be a requirement for you to attach copies of the x-ray or ECG results, so if you have a good history with your local GP perhaps they could save you some money, state no abnormal findings for the x-ray and ECG, and only do the bloods (provided that they are fairly sure that you don't have any chest or heart problems). |

Yea I know the form you mean, there's one on the visaforchina Australian website that asks for some of the additional bloods and no copy of the chest x-ray but the one I have does ask for the chest x-ray to be attached and for all the extra bloods including some not on the form i think you are talking about. The forms on the US and Australia consulate website are different to each other and the one I've been sent is different again. I need to let them know later this morning before the blood test at the lab. Might call the visa centre in Edinburgh in the morning and double check the form. I probably will end up paying the extra to be safe as this has cost too much money to screw getting a visa up. |

I think there is still a standardized form, certainly for government institutes-hasn't changed in years, unlike many other things! Medical form I was sent in June exactly the same as the one sent to me 12 years ago. If this is completed with all evidence included, such as the x-ray, it's still true that another medical will not be needed in one or two provinces-Guangxi, where I am, for example. So it's worth taking the time to make sure you have the exact form required-completed, with all evidence attached. |

The one I have is the Belgian one you posted there. No mention of having to attach copies of chest X-ray. |

My Dr just filled in normal in that place for Xray.
I took the films with me.
Not requested though when I went to the clinic in China for the in-country medical.. |
