Here is a link to a fillable visa application form (Q1). I believe the same form is used for all types of visas. The good thing about this one is that you can complete it online and then print it, which is a life saver for those of us who have horrible handwriting.

Perhaps we could also come up with a good list of websites for Visa agencies who have courier services, which apply your visa to China for you rather than you having to travel to the nearest consulate in person.
There are too many to list. Doing google search you can find one yourself.
Any visa agencies you suggest or you strongly encourage travelers to avoid?
