I am living in Canada and I'm applying for a Working Holiday Visa to Japan. On the Consulate website, it just says I need a note from physician stating that you are in good health. The details are kinda vague and my doctor don't know how to write a note.
I am thinking I write the note for him and he can check it over. If he thinks it is suitable, he can sign for it...can anyone help me with this note? Such as provide me with a sample? |

What's so hard about making an appointment with a doctor, or going to a clinic and asking them for a letter that you are required because you are going to Japan and work overseas? It's simple. The doctor with give you the once over, and then write the note.
Don't write the note yourself. Silly.  |

I agree, if your doctor can't figure that out, pretty standard thing really, run, don't walk, to another doctor.
Lots of jobs and sports teams ask you for medical clearance, so this shouldn't be anything new. |

I can relate, actually. We originally came on a WHV too and the doctor's note was a minor snag.
When I went to see the doctor, I told him what I needed/wanted but since I didn't have a family doctor, (even though I had seen this particular doctor a fair number of times in the past) it was a little challenging.
He said to me: "Well, how do I know that you ARE fit to travel? What am I supposed to do, run lab tests? Check for diseases? Yeah, I can do that but it'll cost you -- and it'll take time too -- about 6 weeks"
Well, 6 weeks and a lot of cash were two things I had none of. So I told the doctor that he could just give me a once-over, confirm that I don't have any outward symptoms of communicable, contageous diseases, and that in his professional opinion, I would not drop dead or fall into a coma on the plane or upon arrival in Japan. So, he said "Ok" and that much he could do for me... So I paid like $50 for him to write up a letter, and that was it.
Make sure, that if you pre-write something for the doctor to sign, he will be willing to sign it... If you write in something like, "I certify this person doesn't have HIV" or something similar, don't expect him to sign something that he has no knowledge of and cannot guarantee. |
