No Longer Coming to Teach, what should I tell company?

To make things clear, I've decided to stay in the U.S. in order to take care of some medical issues I have. Big decision (seeing as I went through the whole process for the Visa), but I thought it through. What is the best thing I can tell the company? They are expecting me for training on the 30th of March. I don't want to tell them that I am handling some medical issues, because then they may not ever hire me again. I don't plan on going to work in September 09 (the next hiring season), but it never hurts to have the door open.

What can I tell them? The visa will remain "open", I guess, until the expiry date. Should I tell them that I want to cancel the Visa? I know they can't because it's an immigration issue, but it would help me to feign some humility. Anyway, what would be my best move?

I think the best move would be to tell the truth. If you are cancelling with only a day or two before your training then they won't be happy unless you have a good excuse and I can't think of a better one than a "health issue".

If you're cancelling so close to the time you are due to be there then that is as good a reason as any for them to deny you a second chance but if you explain to them the situation they may be more sympathetic.

What are the other options you are fishing for here? (Telling them that a very close relative has died and you'll have to go the funeral? - I wouldn't do that for a number of reasons, on taste grounds as well as the fact that you'll have to remember the lies you originally told).

First thing, you've probably got no chance of them employing you again, so put that aisde. Simply tell the truth: send them email saying that a medical situation has come up with a family member (no need to tell them who!) and that you can't leave. Apologise, and leave it at that.
