I received an offer in China and need to be there in a month. My employer sent me a medical form to be filled out ASAP & sent back before they can issue the paperwork required for a visa. Where can I go immediately to have this done? Do specific hospitals have an arrangement with the Chinese embassy? I e-mailed the embassy but have not received a response yet.

Try calling a local hospital and tell them what you need. Very often emergency room physicians can do these forms. I'm assuming it's your health check-up for the job. |

I called the Chinese embassy and they weren't able to help. So I went to my doctor's office and make an app't for Wednesday (the earliest available). I hope that it's OK to have the doctor fill it out. I just hope there's enough time to get the lab results back, etc. |

Call around to the local urgent care centers/walk-in clinics. Often, they can do x-rays, EKGs, and the blood work on the premises. If you go to your GP, there is a good chance he'll have to send you out to get all the tests done, and that could really slow you down (if, like me, you are currently working). |

In my area the walk in clinic wants about 800 in Cash - US money - no checks no credit cards. Might even be more when they are all adding it up. That is for all the tests, x- ray, paperwork so on and so forth. If you can find one for 300 - lucky you. |

Mine was $230, for absolutely everything, and I live in a high COL area. |

Try your local public health or find out if there's a clinic that contracts with employers for exams. |
