Do we take the X ray and bloodwork documents to the consulate to get our z visa? or just the medical form? |

The embassy/consulate just wants the medical form. There is a place on the form for your doctor to write in his/her evaluation of the tests, ie "ECG normal", "HIV negative" etc. |

but if you take the xray to the clinic in china when you redo
the physical, you may be able to avoid getting another xray. |

Good point, and the lab I went to will provide the test results in CD form. |

Question - so the medical form is usable for a year - does the consulate return it to us then? to take with us to China or to use for a different job or visa if the first job falls through? My doctor already said they will charge extra if I need a new form and I might have to do the exam again if a month or 2 goes by - again doctors see dollar signs. I will definitely make a copy of it before mailing to the consulate - but I think consulates only accept originals (real ink signature?). I also didn't know the form had to be front and back since it prints out as 2 pages - so now I have to get a front/back copy. Concerning the pic - just like a passport pic or just the size the same - for some reason when I renewed my passport the post office rejected some pics I had done at CVS store - said the face/head sized was off and CVS refused to give back my money - they are actually better pics IMO than the ones they did accept. Anyway I would just as soon use them if I could. |

The consulate does require original documents and they will be returned to you. I'd be surprised if the pic was a problem. |

i took all my original docs to the consulate today.
while there, the clerk made copies of my invitation
letter, work permit and health certificate,
returning the originals to me. all they kept of the
original paperwork was the visa notification (which
in my case just said to apply in thailand - no city
designated) and the visa application form. they
only wanted one photo.
i have the original health certificate, but it will
most likely be useless in china. the uni has already
informed me i will need to go through the whole
exam process a second time, which they will pay for.
the offered me one day service (returned this afternoon)
or 2-3 day service (pickup tomorrow morning) for
an additional fee. |

What indication do you have that the exam results are "good for one year"?
Who told you that the form must be submitted as "front and back" format?
Why did you have passport photos taken by someone other than the passport agency? The passport agency requires that you use their photo.
If you went to your post office to renew your passport, why do you say that it rejected "SOME pics" but apparently accepted otherS. [Note the plural]. Only ONE photo is required for your photo.
A LOT doesn't add up in your post. Why is this? |

Miles in the USA - photos can be taken at other places besides the passport agency - which in my area is a Post Office. As it turns out the PO wanted to make money and I did have it done there.
Medical exams are expensive in the USA - certainly a person is not going to want to spend 500 this month or more and then another 500 or more next month (USA currency) if for some reason the jobs falls through.
The USA is updating the way photos can be taken - they now use computer imaging to identify - so the measurements of the person must be exact and the background must be exact.
When a person prints out the medical form it prints out as 2 pages - apparently when we send it to the consulate - it must be 1 page - front and back.
Actually you need 2 photos for a USA passport - 1 is attached to the new passport - that is the way it is in the USA.

If we use a visa service - I wonder if the medical form is returned to us?
Did you get your forms apostilled and or notorized - some people say it is required - others say it isn't required.
Did you take your xray and blood lab report with you to the consulate? |

got a student visa in houston 5 years ago through an agent.
medical was submitted as two pages. original was returned.
chiang mai consulate yesterday accepted the two page medical.
they don't want to see the xrays or the lab results. they are
not doctors, just clerks. all they want is the medical report
with an official stamp of some sort.
got my z visa this morning. 24-hour turnaround. no questions
asked.
have never had anything notarized for visa applications.
sandy, if you go in person, just take everything you think you
might need. and some things you're sure you won't. avoid
extra trips.
really doesn't matter if the medical is not returned to you. it
can't be used in china. you must do the medical exam again.
it should only cost about 250 RMB in china, but your employer
should pay for all fees in china.
take the xray with you to china if you want to avoid the
additional xray exposure.
pharmacy type photos have always been good enough
when i've applied. |

You really didn't answer my questions.
I didn't have any problem with sending my medical form to the Chinese consulate printed on two separate sheets of paper. It was returned to me with my visa affixed to my US passport, unmarked, and (apparently) unnoticed. |

I'm in China now. Has anyone done the physical here? Before I would just ask the school to provide me the address of the place I should do it and I would pay to do it one time. Now I am told I must ask in HOng Kong and I am afraid they will tell me to return to CAnada. No one knows for sure about anything. |
