Health Check for 2nd Contract in China?

I am close to finishing my first contract, so I currently have a visa and residence permit. My contract ends in January and I am talking to other schools about teaching next year.

One school sent me this:

According to new government policy, the school also requires the following:

1. A digital photo (colour photo of your head and shoulders sent in a JPG format
2. Residence address and phone number
3. Mailing address and contact telephone number
4. Specific start and end date of higher education and major
5. Specific names of past employers and start and end dates (MMYY) working for each employer
6. A scanned copy of a Physical Examination Record for Foreigner. I will attach this form with the letter of appointment. The completed form should include a personal picture and physician's signature. Please bring the original Physical Examination Record with you to China.

I am curious about number 6. This sounds like what I had to submit to Chinese immigration before I came to the country. Do I actually need to do this again? I haven't left China since I came here.

What type of health check is required for someone with a previous visa who is starting at a new school?

It should have an expiration date. My first and second health cards were good for one year. I've heard that in some places, they will be valid for six months forthwith.

Did you leave China in between the first and second contract? You needed a second health check?

No, I did not leave China between exams. Somewhere in the booklet (I think it's on the first or second page) there is an expiration date. Sometime before that date, you have to get another exam. It's basically a physical: blood and urine test, x-rays, etc.

When I switched schools last year, another health check was not required. My new school is in the same province.

an annual health check is required by law, whether for employment
or for attending university. this is often not enforced, especially if
you are staying in province.

they're quoting the regs for someone out of country....asking for
the "Physical Examination Record for Foreigner." once in country
you redo the physical and get the little brown booklet.

ask if they'll accept a scanned copy of your current health certificate.
if not, just redo the physical. it's the same deal you went
through when you arrived. takes half a day (xray, bloodtest,
ultrasound of yer guts, eyetest) and only costs 250 rmb.

In Chongqing the health check is required whenever teachers renew their contracts and apply for a new foreign experts certificate and work permit. If you have a health check from another city then you must do it again here. The health certificate is only good for six months. This applies even if you have not left the country.

I stayed in the same province as last year just a different city. I had to do the health check again before I started. Simple procedure. 1 year expiration.
