So many people have been asking for forms they might need. I have uploaded some that I have. If you can think of any other you might need, let me know, and I will add them. Hope this helps people.
Letter of Release:
Fingerprint Page for RCMP (Although it might be incomplete as the woman at the Canadian Embassy couldn't scan a legal size paper, she does have an original if you need it.)
And E2 Health Statement.
E2 Visa Application
Immigration Form: Alien registration, Extension of Sojourn, change of address, alteration or addition of workplace, and multiple or single re-entry visa.

Ying, what are you planning? If I am not mistaken, and you aren't going home, all you need is a Background check from a local PD, and would have to authorize someone from home to do it. It takes less time than the RCMP check, and the Canadian embassy will notarize it. The only reason you would need the RCMP check, is for the VSS. And if you can side step the Korean embassy in Toronto, by getting this paper notarized here, then the simple background check should suffice.
However, if you want to get a copy of the legal size, contact the Canadian Embassy. They have it on file. |

How about the FBI finger print card?

Might also want to put a sample "Affadavit" from the US embassy, or jsut a general affadavit that someone can download and take to the embassy.

Just a note - I got a fingerprint form exactly like the one above filled out and stamped by my local police station in Korea and the local RCMP in Canada wouldn't accept it. |

That sucks. The Canadian embassy lady told me that she received that form from the Edmonton detachment, and said that it would work. I just read on another thread that you can apply for a check from truecheck and they will send you a check from a police station. Seeing as you are in Korea, you will not need the VSS.
I got my brother to get my check for me. I scanned 2 pieces of I.D. and an authorization letter for him, and he was able to get it for me. |

I got the same form from the RCMP in BC but they wouldn't accept anything that didn't first go through Ottawa (which takes months). The Canadian embassy also told me that they wouldn't accept a truecheck form. I ended up taking my holidays earlier than usual this year, before my contract was up, just so that I could get the check done in person in Canada. |
