Interesting changes to the Self Health Check form

I re-signed my public school contract today. The self health check form has changed quite a bit from what it looked like when I was still back in the US. It asked:

1. How I rated my own health (poor, good, excellent).

2. If I had been to the hospital over the course of the last year for any illness.

3-8. HIV and drugs.

Shouldn't they just, at this point, change it to "self criminal record background check"?

I wonder if there are people out there (who don't read any forums about moving) who think that the "medical check" is the self health check... especially those who are worried about the drug part.

"Oh, I just say no and I'm clear?"

Anyway, where did you get that new copy? The ones I'm seeing are pretty much the same. Maybe it hasn't made the rounds yet? Or just a specific consulate is using it?

I'm in Incheon. I just thought it was interesting because last year, the one I was sent while I was still Stateside definitely focused on actual health questions. The upside is, this copy didn't have the questions about mental health or tattoos, which I think should have been done away with. It definitely seemed like a special version for renewing though, because it asked specifically if I'd been to the hospital since I'd been in Korea with an illness.

The document requesting the med check from the hospital also had in bold and in English "including HIV and drug tests". They're not beating around the bush this year....
