I've been trying to download the pdf files for the Applicant Information Form, Credit Card Payment Form and the FBI Identification Record Request Checklist off of their website for about a week now and keep getting broken link and blank screens. Anyone else had this issue? Does anyone else know where I can get these forms?

I went to the City Police Dept. today to try and order the crc from there. They were able to do my fingerprint cards, however, they only do a local background check - it doesn't even cover the whole parish, just the city of Slidell. I'm going to have to mail it in, but I need all the other forms.

Any help would be appreciated.

Btw, the FBI charges $18 per copy. Since I haven't decided on where I will be, should I order several copies for all the different schools I am considering, and do I have them send them to my address and then send them to the schools when they ask for it?

You should just need one, but get a couple to be on the safe side. Have them mailed to your home since you have no idea when you'll get a job and when they will actually be mailed to you. It's probably best to wait until you have your documents or are near to having them before searching for a job. Nobody will want you until you have them in hand.

The CBC is for immigration so you can get your visa, it's not for the school. Hopefully you'll only need to send your documents in once, but get a few of everything and hope for the best.

So if I am getting an F-4, I don�t need a CBC?
