There are definitely many problems with this survey- your wording and choices leave a lot to be desired. One BIG problem with it (and, yes, I did complete it honestly) is that you do not take into account MARRIED couples-- Yes, I have sex, without a condom, whether I'm under the influence of alcohol or not-- WITH MY WIFE! I hope this data doesn't get skewed to report unsavory practices of the 'foreign teacher' community... If you need help with preparing a proper survey, feel free to contact me-- This type of work is what I did prior to my ESL 'life' in Korea. It isn't hard, but you need some basic survey methodology criteria before you collect this type of data for it to mean anything. This survey doesn't come close...
