It's a good question. I never thought about it before.
If you don't mind, I'd like to ask a different but equally valid question.
How is it Lois Lane never saw any resemblance between Clark Kent and Superman? |

I'm embarrassed to say that I thought there was some cartoon or something where Casper dies, in the same way that Superman and Captain America died. Then it dawned on me that he was a ghost and ghosts can't die.
I thought that he'd probably been some sickly kid with cancer or something. Although I like Lisa Simpson's theory. Or maybe it's like some Man in the Iron Mask type thing (I've actually never read the book or seen the movie so maybe the comparison is way off) and Richie Rich locked up his twin brother Casper Rich so he could control the fortune himself but then there was some kind of incident and Casper was gagged and ended up choking on his own vomit or something. |
