Does anyone know where I could find a class evaluation form to give to my students? The kind of them that asks them to rate what is most important to them in learning English, for example, or gives them a chance to evaluate the class. I can make one, but if there's a good one available, I'd appreciate knowing about it. thx...

PM me and I can send both an English and a Korean one.

Not the best but it covers the basics, end of course teacher / course assessment.

As an aside, it must be noted here that in my opinion, students should always do evaluations in their native language!!! So important and I have been so surprised the last few years at the number of teachers giving an evaluation form which is written in English.

The course/teaching evaluation form, is not for learning/teaching but purely for administrative purposes. Also, make sure you are out of the room, be it grade 3s or adults.... and the evaluations go directly to those concerned and not to the teacher. I know this is basic stuff but I'm surprised at how many teachers don't do it!

I think you were looking for survey forms. sorry, my reply was about post course assessment of the teacher....
