Is there a form for the medical check? Where can I find it?

I want to go do the medical check thing. Is there a form that I need? Where? Can I go to any doctor?

The medical check has to be done in Korea. Immigration published a form for the "E-2 Applicant's Health Statement" which is a questionnaire to be filled out by the applicant. That form can be downloaded here: http://www.chicagoconsulate.org/data/file/form/1171484886_768cdb0a_Applicant5C27s+Health+Statement.pdf

I haven't seen any form for the medical check itself, though. Immi was supposed to publish a list of approved medical centers, but that also hasn't happened yet to my knowledge.

So, how have people been getting their medical check?

This is crazy. I am trying so hard to do everything I am told I need to do and its dead end after dead end.
