A Health 'Statement' or a Medical Examination??

Today, the company that recruited me for my present position sent me an e-mail saying that I needed to fill out their health 'statement' form. This came immediately after I went to my school early and told the director I had a doctor's appointment ( which, instead of being in anyway concerned about my health, proceeded to tell me to hurry...it was Monday, I have no house phone or cellphone, so I could not call her...had to walk to the school and tell her in person ). The 'Health Statement' form was not an examine, but a questionaire asking me if I had such and such health conditions...and was prefaced by a statement that any false information would lead to dire consequences along with deportation. However, I've checked the new regulations, and as best as I understand it, I'm supposed to go to a government approved hospital for a complete medical examine. If I must do that, why is the recruiting company requesting a 'health statement' form...signed and dated by me ( mailed to them directly...can't be faxed ) instead of sending me to an approved hospital for the health examine? Do I need to do both? I'm just completely fed up with this circus and just feel like telling all Korea to kiss my @ss and call it ice-cream!!
