I'm pretty sure this will count as number three.

Just reported that Isaac Hayes has just died. Yes, he was one of the founding fathers of R&B and rap, and wrote the theme for Shaft. But, he was also the voice for Chef on Southpark.

I've never found this. One celeb dies and then chance sometimes has another celeb dying within a reasonable time. And then every notes deaths come in threes. And then everyone forgets about it, never enumerating the third death.
