Hi,
For a school project I am conducting a quick questionnaire on coffee establishments in Canada. It takes about 1 minute to complete the questionnaire and it is anonymous.

Your survey should be more professional. It's written like a high school kid who learned English via text messaging.

Why have a "dead" option in age range? That's just prompting users to not take your survey seriously and your later questions give many opportunities for smarmy ratings.
Why !! in many of your option? That's unprofessional.
Weak-crappy? I might agree some coffees are weak but not crappy. Some are crappy but not weak. They're measuring two different things.
They're everywhere vs they're on every corner. Isn't that roughly the same thing?

I'm not sure what kind of course you're submitting this for or what the standards are your teacher will hold you to but this is a huge fail in any reasonably rigorous university level undergrad course on experimental design.
