I am aware that when you visit international clinics in Korean hospitals, the cost is more for the consultation with the english speaking doctor.

My question is......if he/ she sends you to another specialist (allergist/ ear nose throat etc) , do you pay more for their services also?

Curious if I should just skip the international doc and head straight to the specialist.

Most of the hospitals where international clinics are available are more expensive. So, if your doctor refers you to a specialist, you will have to pay more for their services, too.

I once went to a clinic because something entered my eye and I had an allergic reaction. The doctor said I would be okay but to be safe, I could see an optometrist. He suggested I visit a different hospital since the services would be much cheaper there.

Network with friends and find English speaking doctors who have their own offices. Hospitals can be expensive and aren't any better than a good English-speaking sole practitioner. I've got my ENT and gyn and neither costs me more that 4,000 won per visit.
