Where should I have the 6616 form mailed to?

I am mailing out the 8802 form to request for the 6616 form. I only have the SMOE address or my recruiter address. The form says to allow a full 45 days for it to be mailed back to me.

Should I take my chances and have it mailed to my home address (NYC) hoping that it'll get here in less than 45 days or should I get it sent to the SMOE office? I have to leave by the 27th. I'm worried that the SMOE office might lose it or that the IRS will delay it for whatever reason. Anyone know what is the best process for this form?

You are talking about leaving home by Feb 27th? It takes about 45 days for it to arrive, and that is after the people in Philly get the 8802. There are about 16 days left before the 27th. I'd send it to whichever address allows me to get my hands on it and take it to the tax office in person. That would be North Carolina. You can later have someone at home mail it to your school. From there, you can take it to the tax office.
