I had to do it, and have yet to get the official document. Yet I leave on the 18th.
Basically, you fill out a form (8807 I think?) and send them a MONEY ORDER no personal check where they then send you an additional form, haven't received it so I can't tell you its number. Then you are suppose to be good.

Hope this helps but my guess is that you are as lost as I am about the IRS form.
