I am faced with something that must have come up a lot, but I am not sure how other teachers handle this.

My contract is up on September 15. I have another school lined up who wants me to start on the 18th.

They want to transfer my visa so I don't need to get all the paperwork ready and both schools can avoid paying for a flight. However, how do I address severance, and what date should be put down for the release letter?

Can the contract end on the 15th, new contract start on the 16th, and I still get my severance? Or, will immigration say, "On the 15th, your visa will no longer be transferrable", therefore making it necessary to submit all the paperwork again?

I didn't have any problems last year, but it wasn't at the end of a contract period. I don't want to run into problems.

I've already done this. My situation was a little different because I switched employers. I had to get a letter of permission from my employer to add or alter place of employment. It stated the last day I would be working. I needed my new contract signed stating the date I would start my new job. I was allowed up to 14 days inbetween leaving one job and starting the next. There were a few other things they wanted, like a photograph, and I think they wanted to see my diploma again.
In any case you are allowed 14 days between jobs.
