I used to have one. Its great for a two class filler.
Class 1, get them filling it out.
Class 2, get them sitting in a big circle and play pass the parcel with 2 dice. Once the music stop, the two people with the dice roll. Highest roll has to say stuff from worksheet:

For instance
1 = name, age, birthday
2 = name age birthday, friends
3 = name age birthday friends, what time they get up
4 = name age birthday freinds what time they get up and like/dont like
5 = name age etc etc etc and hobbies
6 = whole worksheet.

Anyway, looking for something with enough stuff. Doesnt have to follow that template, but something pretty would be nice. I make the ugliest worksheets on the planet and have no skill at all with word (probably because its in korean anyhoo), so im being lazy and trying here before i bite the bullet.
