I was born in Belarus, but have a Canadian passport. No problems getting my 3 E2 VISAs thus far.

There's a specific age by which you have to have emigrated to North America (and studied in North American schools). I can't remember what it is, but I think it's about 9 - 12, somewhere around late elementary school age. Any later than that, and I'd worry about getting the VISA.

Russia Township, Minnesota
Russia, Ohio
Russia, New York

Some place in the application material, use an address in one of these places (previous address if available). Use the one that is closest to your home state. No, I am not joking.

20 years ago Russia was the Union of Soviet Socialist Republics. What does her passport really say?

It may still say Russia. Boris Yeltsin was the President of Russia while a member of the Soviet Union, so the passport might still use Russia or whatever republic was representative.

No, Russia was part of the Union of Soviet Socialist Republics. It was (theoretically) a union of different republics.
