Just thought I would share my most recent lesson plan from my blog. It focuses on the Past Simple question forms as well as the affirmative forms. Please feel free to use this and provide any feedback/comments.

Good to see people posting lesson plans up and not guarding their material jealously from their co-teachers and others, like some of the posters on Dave's. Looks like a good activity, though next time I'd post the level you think it's aimed at as well. The first activity looked pretty tough, Koreans really struggle with mixed up word order exercises like that and I'd say it was minimum intermediate level. Students are usually introduced to past simple at elementary level so it's important to make it clear that the plan is not aimed at that level.

Edward thank you for the feedback. I agree that the activity would not be easy for Pre Int students. I hope to have more lessons uploaded on my blog in the future. If you also view my previous posts on my blog there are some other lesson plans.
