I expect this question has already been answered a lot on this forum, but just to save a little time searching for the answer, I'll post it again.

Is apostilizatiion required with the United States FBI criminal record check? If so, how does one go about applying for the appostilization part?

Yes, it is required. From the FBI website:
The CJIS Division will authenticate U.S. Department of Justice Order 556-73 fingerprint search results for international requests by placing the FBI seal and the signature of a division official on the results if requested at the time of submission. Documents prepared in this way may then be sent to the U.S. Department of State by the requester to obtain an apostille if necessary.

Send $8 and the cover letter (found on the US state dept. website) to:
US Dept. of State
Authentications Office
518 23rd Street NW
SA-1
Washington, DC 20520

Thanks for the info. Unfortunately, it looks like I may have to repeat the entire process. There was no box to check that I saw on the application form to make such a request. Also, if I understand the procedure described correctly the criminal check has to come back to me and then be sent by me to the US State Department for the apostille. Big drain on time and money. But at least FedEx will be happy.

I find that to be both totally absurd that a government form would fail to have a checkbox for a service that may be requested with that form and completely believable at the same time.

Now, I am looking for someone who has recently and successfully gone through this process to reply to this post with their personal experience of who they did it. I'm not sure what the dates are instituting these new and difficult hoops to jump through are but I'm guessing that someone on this forum may have successfully cleared all the hoops by now.

PS. Don't ask for any info from the US Embassy in Seoul. The ones I've spoken with there do a most impressive imitation of being completely brain dead.

Perhaps not. Email the FBI criminal records division (I assume you are still waiting to receive your CBC?) And ask if you can either request the seal & signature by email, or if you can fax in an official request. They always reply to emails. Their address is: [email protected]
Yes, it is a lot of time and money. FedEx overcharges, but they are fast. The US Postal Service (not ups) is way cheaper. Not so fast, though.

Are you in Korea or not? If you're in Korea, you may go to the Embassy to swear an affidavit, as the Korean immigration office accepts that in lieu of an apostille (or if you're coming to Korea very soon). www.hikorea.go.kr read through there.

Also, don't be afraid to contact your Congressperson to help speed the process along.

I think it's a bit ridiculous as well. It seems a lot like scribbling "Please mail me a refund check" at the top of your tax return.
