I'm trying to fill out the visa application form to send to my embassy after I get my confirmation number. I'm confused about some of the questions.
One of the boxes (box 12) asks for my national identity number. If I'm from the US, is that just my social security number?
Under the passport section it asks for classification (14) and gives the options to check DP, OF, or OR. I don't see any of these on my passport. I see a "P" under Type, but thats it.
I also can't find the "place of issue" (15). What exactly are they looking for? State? City?
Also, the last box (35) asks for Guarantor or Reference in Korea. Is this my representative/recruiter? Do I have to fill it out?
Any answers would be appreciated.

box 12 is passport number
box 15 is what it says on your passport for "place of issue" or "authority"
box 35 is for a different kind of visa than E-2, you don't need it |

DP = Diplomatic.
OF = Official.
OR = Ordinary.
Your passport is OR. |

DaHu, I thought box 12 (national id no.) was the same thing, but then I read box 13 and it asks for my "passport number" so I doubt they are they same thing, right? Any other ideas?

I had many of the same questions as you OP when I was filling out the application. Google can help with the DP, OF, and OR abbreviations, but CentralCali's explanation is right. As for the National Identity Number, at the NYC consulate, I was told to leave that section of the application blank. I was also told by the consulate that your Guarantor or Reference in Korea is your employer. I applied for an E-2, but maybe DaHu is right in that it's probably unnecessary.
I recommend confirming any advice (including my own) with your nearest consulate. While it may be challenging to speak to a consulate representative on the phone or in person, it's better to be safe than sorry.

Common Visa Application Questions
o Section 3, Leave Blank
o Section 12. National Identity Number- Leave blank
o Section 14. Classification � OR (ordinary)
o Section 15: Place of Issue- USA
o Section 18, 19, 20: Employment presently in the USA
o Section 26: English Teacher
o Section 27. Desired length of stay � 1 year
o Section 30. Address in Korea � you can use the school�s address
o Section 32. Who will pay for your trip � Employer
o Section 35. Guarantor in Korea � the director of your school
This is from my local consulate, but I would check with yours to be sure. |
