Does anyone have a good format for teaching evaluations? I teach at a university, and the evaluations done by the students (designed by one of the teachers) were not really effective last time, and I would like to be able to get evaluations from my students, and to help design the next department evaluations.
If you have a simple evaluation form for students to evaluate your teaching, I would love it if you wouldn't mind passing it on.
