Need fingerprints. Finger print paper???

This is a question for anyone who recently had their finger prints taken.
I called the local police station to say I need my finger prints taken for a job in Korea. I was told that it is $10.81 for each copy, but I had to bring in my own form. Do I buy card stock and down load the form from the internet? Thank you.

Yes, you will most likely have to pay.
Yes, you will need to print this: http://www.fbi.gov/about-us/cjis/background-checks/standard-fingerprint-form-fd-258 out on cardstock.
