I need to get a health check signed off by a US certified doctor. It is for applying for a US state high school teaching certificate (will complete the online program soon). I have seen many people posting here about the health check for Korean Immi, but this one needs to be a US certified doctor. The form has no details abut the actual health test, it just has:
Signature:
Date:
State licensed:
Physician number:
Anyone ever get anything like this signed off in Korea?

The consular section at the embassy used to have a list of doctors (needed for US immigration visas).
I would suggest trying that.
