EPIK Application Questions [Need Advice]

There are a few questions on the EPIK application which make me wonder.

1) ⑭ When and for what reason did you last consult a doctor?
Honestly, I dont remember I know it was last year, however I dont currently have medical therefore it will be extremely difficult for me to look up. How precise does this need to be? do they even check?

2) ⑬ I understand that all successful applicants must take a medical exam in Korea in accordance with the requirements of the EPIK program. If the results show that the applicant is unfit to be an EPIK teacher, all costs for entry, stay, and departure will be borne by the EPIK applicant.

ADDING ON TO THAT
If necessary, are you prepared to undergo physical tests to verify the answers given in the Self Medical Assessment?

I did some research, and it looks like they are going to test you for basics, such as TB, HIV\AIDS, and weed.

I never had and do do any of those, is that the extent of the testing? Or are they going to ask me to run a mile or some weird stuff.

Are they able to check medical records? I doubt that due to HIPAA laws and it even being extremely hard for me to get my own records.

3) NON-TEACHING WORK EXPERIENCE List from the most recent employment. Add more rows if necessary.

I held quite a few jobs in the past, are they going to verify employers? I have no problem if they do, its just there is no ability for me to provide the individuals who could speak to who I was and give them the information they need, as I was in positions of management. In short, how often are these checked?


Answering these three questions will take a load of my mind, I would really appreciate some help from someone who can answer this, as I really want to teach.

Finally, anyone got some help with a badass lesson plan for the application? I am having trouble coming up with something which is EPIK Application worthy.

No they won't check, approximate the time

That's if you give additional information, which you shouldn't be doing unless they will find out about it by looking at you.

Only a drug test and communicable disease test. Everything else, unless you are just outrageously physically unfit, you won't have problems with

No, just the physical and that's it.

The ones that are directly relevant to teaching, I am sure they will. The others they won't.
