I'm filling out the Application for Visa form and am slightly confused about 2 things.

First, Do I write in the visa issuance number at the top of the page? Or is that for them to do at the embassy?

Second, do I affix my passport size picture to the document or do I just send it in with the document and they will attach it there?

Thanks all. As soon as I get answers I'm off to the post office.

Didn't get a definitive answer to the pic question...so I went ahead and glued it. As for the visa issuance number, you do write it in before sending the application to the consulate.
