I am probably going to go in a quit today. Stress is causing me health issues.
I will negotiate to stay two more weeks if they rewrite my contract to end early (in two weeks) and give me a less stressful condition during for those two weeks. If that can't be done, I'll probably just quit today.

This has probably been asked before but:
If I quit, can the employer put a "black mark" on your immigration file so that future employers see that you quit?

Can a fairly powerful employer put a "black mark" on your immigration file so that immigration won't let you work other jobs.

Just want to check before I quit.

1) NO.

2) sort of....

. . . 2a) immigration CAN deny you the opportunity to take other employment until the end of your current contract period.
. . . 2b) Once your current contract period is complete then NO.

3) immigration will give a black mark that will impact future visa applications IF you violate an immigration law (overstay, illegal work, etc).

2a is very outdated. They stopped doing that a while back.
In theory, they can deny you a visa or the visa issuance number for any reason at any time but simply not finishing a contract is no longer an issue. If you have a new set of documents, you're good to go.

I would not be looking for work until the dates of this contract are over.

I've never quit before on bad terms.

Do you just tell them and walk out? Is there an "I quit" form?
If they ask me to sign anything, of course I wont, I'm just telling them and going.

If you don't need the LOR, owe them nothing and they have no deposits from you then just walk at your convenience.

Notice or notification is NOT (by labor law) legally required to quit and you cannot (as an employee) legally be held responsible (again under labor law) for non performance of a labor contract.
