So, can anyone tell me if either/neither/both of these are wrong?

A: Well, should have Greg's boss fired him?
B: Well, should Greg's boss have fired him?

Also, should have, ought to have, could have indicate that actions and states were advisable in the past...but is might have too?

Any help would be appreciated, this is all starting to look and sound the same to me.

"A" is wrong. "B" is correct.
