I finally found out how this works. When you get the F4 visa, you can also sign up for the government sponsored medical insurance program native Koreans enjoy. It pays for most doctor visits except for the costlier ones. I don't know the details of the coverage but basically you have to pay something around 40,000 won per month.
I did not know this and was covered by my US employers US insurance system. Of course its a pain to use that system because you have to get all the dolcuments provided by Korean hospital and pharmacy to be translated into English for reimbursement through the US system.
Now that I am not working I went to sign up for the Korean insurance and was told I have to pay for the time since I received the F4 visa upto maximum of 3 years. Since I received the F4 5 years ago they said I have to pay 3yrs worth which is 1,650,000 won.
However a way to avoid this is to get employed at a place that will register me to the government system. Freelance kind of work would not work allow access to the Korean insurance system.
Another way is to have a business and have employees. Then I can use the employees registration as way to register me into the system without paying the 3yr back payments.
I don't think I have any other options. If anyone knows of any please let me know. The foreign insurance companies in Korea only insure the costlier additional charges not covered by the Korean insurance system.

| Very true. Same for E2s and F2's. You owe them money but dont have to pay if your employer is already providing insurance and you just get written in under their insurance. |
